This year we combined old school with new school.

To fill out your registration form, you will need to download and save the pdf file that is in the link below, once you have downloaded the file and saved it to your desktop.

Open the file and fill it out. Once this done rename and resave the file. You will need to rename the file with your company name.

After that is done, you will need to to email the completed form to the ERA office email address.

If you do not wish to fill out the form on your computer you can print the form out and mail, email, or fax to the ERA office.
